It is very important to follow the recommended dietary guidelines after undergoing bariatric surgery. These guidelines were carefully designed by your health care providers, with the goal of limiting the amount of calories you consume, while providing balanced meals that help prevent nutrient deficiencies and preserve muscle tissue.

How To Get Enough Protein
For a lot of us, getting 60 gs of protein was hard before bariatric surgery. After bariatric surgery our physical capacity to take in food is extremely limited admittedly, kinda the point of bariatric surgery, duh, but worth restating, indeed, celebrating every now and then and restrictions around the timing of food and drink dont exactly make things any easier.
And then theres the GERD. Heartburn. Acid reflux. Whatever you want to call it.
The point is, getting to the protein promised land aint easy, and we all have to find our own path to protein peace and prosperity. But find it we must.
For me, its the protein supplement.
It comes in many different forms. Whey. Soy. Casein. Isolate. Concentrate. Powders. Liquids. Gels. Shakes, pre-mixed and ready to drink for your new, fast-paced, on-the-go lifestyle, or powdered for home use. The choices are endless. Whey protein isolate is the preferred type of protein for bariatric patients because it is the easiest to digest. Most bariatric-specific protein shakes will be 100% whey isolate or primarily whey protein.
Ive come to depend on the shakes because theyre twofers not only do they deliver a power-packed, protein punch on demand as much as 30 grams they also count as fluids in my ongoing pursuit of 64 ounces per day. Suddenly its a lot easier for me to get in that 30 grams over the course of a day. Your results may differ.
So I got that going for me, which is nice.

Stage : Soft And Moist Foods
This stage of your diet will begin about 2 weeks after your surgery and your follow up appointment with Dr. Balsama, lasting 2 full weeks. These foods should be easy to pull apart with a fork, and each meal should be about ½ cup or 4 ounces in size. However, you should stop eating as soon as youre full, even if this amount has not been reached! Examples of soft foods to eat during this stage are:
- Lean proteins like slow cooked chicken or pork
- Canned chicken, tuna, or crab
- Egg whites
- Over-cooked vegetables like zucchini, squash, cauliflower, or mushrooms
- Soft fruits like melons, peaches, and avocados
Continue to sip fluids between meals but remember the 30/30 rule: Do not drink 30 minutes before or after a meal. Continue your daily protein drink and minerals as well.
Why Protein Is Important For Bariatric Surgery Patients
Protein is essential to life your muscles, your organs and your immune system all require protein to do their jobs. Proteins carry the oxygen around your body. Proteins perform the chemical reactions that control how we think, feel, and react. They protect us from disease, and they are quite literally the reason we can move around in this great, big world of sound and color, day and night.
Come up short consistently on your protein daily requirement and youll start to feel it.
A deficiency of protein can cause hair loss, bad skin, bad nails, and worse. The absence of key proteins may even be linked to the onset of diseases like Multiple Sclerosis that can lay dormant for years until they flare up painfully and unexpectedly.

Will I Gain Weight Back After Time
Even with a high average monthly weight loss after gastric sleeve, some patients worry they will gain back the weight later on.
Research supports that when patients do regain their lost weight it is mainly due to not following a proper diet and not exercising. This shows weight gain after surgery is possible but not a guaranteed complication.

Stage : Add Thicker Drinks & Smooth Foods
Begins 1 day post-op to 14 days post-op
Your surgeon will confirm that you have no nausea or vomiting before allowing you to begin Stage 2 of your post-op gastric sleeve diet. Stage 2 lasts from one day to 2 weeks, depending on your surgeon, and includes thicker drinks, pureed foods, andsmall meals:

Remember: No soft or solid food and no drinks with chunks or seeds.

High Protein Foods You Can Eat In Small Portions Include:
- Eggs. You can scramble eggs with different ingredients to add more richness to your breakfast, or you can add boiled eggs to a chicken salad.
- Meat and poultry. Steamed chicken, chicken soup or red meats cooked with little to no oil with steamed vegetables are great choices.
- Fish. Avoid eating breaded or fried fish. Steamed fish with vegetables makes a fine dish.
- Seafood and tuna. Tuna salad or ceviche are good alternatives. When eating raw seafood, avoid dishes with too much rice, since that may spoil your appetite.
- Soy milk and tofu. To use with salads, soups, or a cold glass of milk.
- Cottage cheese, yogurt and other milk products. Great for breakfasts with fruits.
- Beans, lentils and chickpeas. Soups with lentils or chickpeas, beans as a side dish, and even hummus are great options to boost your intake of protein after bariatric surgery.
Protein Supplements Vs Meal Replacements After Bariatric Surgery
A common mistake is to assume protein supplements and meal replacements are synonymous. Meal replacements are much higher in calories and contain carbohydrates and fat which can be counterproductive to your diet. Protein supplements are low calorie, high in protein, and often do not contain any fat or carbohydrates.

What Happens If You Dont Get Enough Protein After Gastric Sleeve
Patients that dont take in enough protein can experience weakness and muscle loss, and this can cause serious problems. In addition to staying hydrated, patients will need to focus on how much protein they take in. Most bariatric surgery programs will recommend 60 to 100 grams per day, depending on the patient.
Phase : Adaptive Or Soft Food Diet For Post
During this phase after bariatric surgery, you will transition to more solid foods. This may include soft meat, fish, chicken, cereals, cooked vegetables, and canned and fresh fruits.
During the soft diet, continue to:
- Incorporate high protein foods into your diet and consume them first at meals.

How Many Calories Should I Eat 1 Year After Gastric Sleeve
After one year on the gastric sleeve pre-op diet, you will then be moving to the post-op diet. You will not be eating the sleeve pre op foods anymore, because your doctor will need to transfer you to the post of ward. This time you will be eating the GI diet bars . These bars are lower in calories and fat than your original bariatric solution. After a little more time on the post op diet, you will then be moving into the lap-band or gastric bypass surgery.
Your gastric sleeve diet after one year should essentially be based on your lifelong commitment to eating a high protein, low carbohydrate and low fat diet. For the first two years post-op gastric sleeve diet after weight loss surgery, make it a habit to always eat carbohydrates first on your plate and fruits and vegetables second. Dont go overboard by eating a lot of empty calories like sodas and coffee. These empty calories add nothing to your nutrition and may even be harmful to your body. Foods like potato chips and sugary desserts should be eaten sparingly after a meal. This will keep your calorie intake down so that your digestive system will be able to process the food you consume more efficiently.
Alternatives To Protein Shakes
If you are getting bored with the protein shakes, there are many different ways to switch up the variety to prevent flavor fatigue! Dont forget that there are clear protein shakes that are not creamy and come in flavors such as lemon, berry, orange, etc. There is even protein water! Another option is buying unflavored protein powder that can be added to plain water, unsweetened decaf tea, crystal light, zero calorie sports drinks, or even chicken/beef/vegetable broth or unsweetened apple sauce. Many different brands sell unflavored protein powder. Some bariatric vitamin companies also sell high protein, low carbohydrate soup packets that you mix with hot water for a nice savory treat.

Week 5 Onwards Healthy Diet
Your new diet needs to be low in fat, low calorie, and portion controlled. Your bariatric team will offer advice and support for your new diet.
Keep introducing new foods and monitoring any reaction to them. Avoid empty calorie foods that do not have any nutritional value. Try to keep to three small meals a day and minimal snacks. Dont forget to keep yourself hydrated.

Gastric Sleeve Diet: Pre
Gastric sleeve surgery is a safe and clinically proven method for reducing body weight. - Drink A LOT of water! Avoid juice, soda, and other sugary drinks.
Reason #: I Dont Like To Lift Weights
Although I tell people you can never out-exercise a bad diet and that diet is at least 80% and exercises being 20% it still impacts the efficiency and the rate at which you burn fat. This does not mean just running on the treadmill. Building lean muscle increases your metabolism because it takes more calories to maintain muscle.
The more lean muscle you have the faster you burn calories. If you are six to eight weeks out and you are eating a proper amount of calories resistance training may be the thing you are missing to take you to that next level of fat loss.
If you do not know anything about resistance training to build muscle it may be beneficial to hire a personal trainer for a few sessions to create a plan for you. If you do not like going to the gym you can create a home workout. If you are reading this and thinking, I dont want to get bulky trust me, you wont. It takes a lot of work and a consistently balanced meal plan to build muscle.
Overall you want to be patient with the process. You did not gain the weight overnight and although for many this is rapid weight loss it will not come off overnight either. Stalls or plateaus are a normal part of the process.

Week 1 Clear Liquids Only
In week one you are limited to clear liquids only. This sounds tough. And it is tough. However, most patients have very little desire to eat. The hunger hormone ghrelin is almost non-existent after surgery.
The part of the stomach that produces the majority of ghrelin is removed during surgery.
Make sure all items listed below are sugar free.
- Sugar free, nonfat ice cream
- Thinned applesauce. Check sugar content.

- 6 months: ~40-60 grams carb/d

In the beginning, your carb intake is low simply becauseyou must prioritize eating protein first. Less than 50g carbs per day iscommon in the first few months.
As youre able to eat a little more, its better to focuson eating the right carbs vs focusing on numbers.
Thinking of a food only in terms of its carb count doesntnecessarily lead you to a good choice.
Fried pork rinds are low carb but does this really makethem a healthy food to eat?! I hardly think so.
Choose foods for what they HAVE , not for what they dont have .

Carbohydrates And Fat 1 Year After Gastric Sleeve
Heres the cool part about structuring diet with calorie and protein prioritization: The rest doesnt really matter.
As long as you are abiding by the first two rules: get your calories and protein right, the rest will fall into place.
Heres why it works this way. Macronutrients each contain calories. So when you look on a nutrition label Total Calories per Serving is derived from protein, carb and fat contents.
Numerically heres how it works out:
- Protein 4 calories per gram
- Carbohydrates 4 calories per gram
- Fat 9 calories per gram
So here are a few examples of how the Carb and Fat portion can fluctuate as long as you get your calories and protein right.
And here is an example of how you can reach the same calories by manipulating only the carbohydrates and fat and leaving protein the same.
Notice how the only thing that changes is how many carbohydrates and how much fat is consumed, but the calories are more or less the same.
As you can see, in both of these scenarios total calories and total protein stay the same, but you can play with and be flexible with your carbs and fat.
